L—蘋果酸是生物體可以利用的形式，它常配以復合氨基酸注射液（手術后重要的營養(yǎng)藥品）中，以提高氨基酸的利用率，這對手術后虛弱和肝功能障礙病人尤其重要。L—蘋果酸鉀是良好的鉀補充藥，它能保持人體的水分平衡，治療水腫、高血壓和脂肪積聚等癥。L—蘋果酸是治療肝病，尤其是肝功能障礙導致的高血氨癥的良好藥物。L—蘋果酸鈉具有食鹽的1/3咸味，可作腎臟病人代食鹽。
L—蘋果酸在食品、醫(yī)藥、日用化工等部門的用途如下：（1）食品工業(yè)：用做加工和配置飲料、露酒、果汁，也用于糖果、果醬等的制造，對食品且有抑菌防腐作用。亦可用于酸乳發(fā)酵PH調節(jié)，葡萄酒釀造中除酒石酸鹽等。（2）煙草行業(yè)：蘋果酸衍生物（如酯類）能改善煙草香味。（3）醫(yī)藥工業(yè)：各種片劑、糖漿配以蘋果酸可以呈水果味，并有利于在體內吸收、擴散。（4）日用化工工業(yè)：是良好的絡合劑、酯劑，用于牙膏配方、凈牙片配合、合成香料配方等，還可以作為防臭劑和洗滌劑的成分。
比旋度 取本品，精密稱定，加水溶解并定量稀釋制成每1ml中含0.085g的溶液，依法測定（中國藥典2010年版二部附錄VI E），比旋度為－1.6°～－2.6°。
【鑒別】（1）取本品約0.5g，加水10ml使溶解，用氨水調pH值至中性，加1%對氨基苯磺酸溶液1ml，在沸水浴中加熱5分鐘，加20%亞硝酸鈉溶液5ml，置水浴中加熱3分鐘，加4%氫氧化鈉溶液5ml，溶液應立即呈紅色。
（2）本品的紅外光吸收圖譜應與L-蘋果酸對照品的圖譜一致（中國藥典2010年版二部附錄Ⅳ C）
